Eligibility

To be eligible for this program you must be certified by the product distributor. Cosmetic teeth whitening must be allowed in your state by non-dental personal.

Coverage Details

• General Liability
  (Written on an occurrence basis with limits up to $1 Million per occurence/$2 Million aggregate)

• Professional Liability
  (Written on an occurrence basis with limits up to $1 Million per occurrence/$1 Million aggregate)

• Abuse Coverage
  (Written on an occurrence basis with limits up to $300,000 Occurrence/$300,000 Aggregate)

• Optional Coverages
  (Coverage to protect your equipment from Damage from certain losses subject to certain policy exclusions - Inland Marine Rate is 2% of your Property Value. Minimum Premium $50. Deductible $250.)
